You are able to request a public record copy of a police report in person by coming to the station and speaking with the individual located at the window. You must be able to provide adequate information such as the date of offense, victim or suspect information. 

​​​​​​​​​​​​Crash Reports are highly regulated and require a Federal Drivers Privacy Protection Act (or DPPA), 18 USC 2721-2725 Permissible Use for unredacted copies, as they can contain personal information about more than one person. They can be ordered online, by mail or in person at the Vehicle Registration office at 4121 New Bern Ave., Raleigh or 12101 Mt. Holly-Huntersville Road, Huntersville.
 

Please note: A North Carolina motor vehicle record (MVR) is a driving record, vehicle record or crash report.  The manner in which MVR information may be released is regulated by the DP​PA and North Carolina G.S. 20-43.1. ​
 

An individual requesting their personal motor vehicle records in person must complete the appropriate forms and present a valid ID. An individual requesting a personal motor vehicle record on another’s behalf must also provide a written, signed, and notarized statement from the individual to whom the personal records belong, giving the requestor their permission to receive the records. 

Any entity may request an unredacted crash report for valid reason under the DPPA. Not all DPPA Permissible Uses qualify to obtain a crash report. 

Requestors who do not qualify for a Permissible Use will only be able to receive a redacted copy of a crash report by mail or in person. 

If you are a governmental agency, or a licensed professional such as a lawyer or private investigator, you will be required to submit the appropriate identification confirming your profession. 

 

Requesting Crash Reports

Online Requests

To order a crash report online, the following information is required:
  • The valid North Carolina driver license or ID card number of the Requestor
  • Social Security number, Individual Taxpayer Identification number or U.S. Visa number of the Requestor
  • Driver’s license number of a driver involved in the crash
  • Date of the crash
     

Before You Begin

The N.C. Division of Motor Vehicles uses an online service, called PayIt, that allows you to take advantage of completing multiple services in one secure transaction. Up to 5 crash reports can be requested in one online transaction.
 
PayIt collects a $3 fee per online transaction plus a 1.85 percent card processing fee, These fees are used to deliver quality services more efficiently with no upfront costs to NCDMV. The State of North Carolina does not collect or benefit from the transaction or processing fees.

Ordering by Mail​​
To order a crash report by mail, fill out the Crash Report Request (T​R-67A) form and mail it along with payment (see fee below) to the address listed on the form. The $3 fee is not required for mail-in requests. 
Please allow 10 business days from the time of receipt for the request to be processed.
To avoid delay or return of your request, ensure that forms are completely and correctly filled out and that your check or money order, made payable to NCDMV is signed.​

Video Disclosure Requests

Release of Recordings [Getting a Copy]

North Carolina General Statute Section 132-1.4A provides the process by which a person may obtain a copy of law enforcement recordings. In order to obtain a copy of a video, you must first initiate a civil action or file a Petition For Release (AOC form CV-270) with the Wake County Clerk of Court. Once the Petition for Release has been filed with the courthouse, a file-stamped copy of the Petition may be served on ZPD at

Disclosure of Recordings [Viewings ONLY]

As noted above, N.C.G.S. 132-1.4A also states that certain persons are entitled to disclosures, or viewings, of law enforcement recordings. This does not include getting a copy of the recordings. By statute, those persons are:

A person whose image or voice is in the recording.

  1. A spouse if the spouse whose image or voice is in the recording consents to the disclosure. Notarization of the request form is required by the consenting spouse.
  2. An attorney retained to represent a person whose image or voice is in the recording. Notarization of the request form by the client is required.
  3. A parent of a minor whose image or voice is in the recording.
  4. A guardian of a minor or adult whose image or voice is in the recording. Guardianship documentation is required.
  5. A personal representative of a deceased person whose image or voice is in the recording. Executorship, Power of Attorney, or other legal documentation required.
  6. A personal representative of an adult person who is incapacitated and unable to provide consent to disclosure. Power of Attorney or other legal documentation required.

NOTE: the statute defines a “personal representative” as a parent, court-appointed guardian, spouse, or attorney of a person whose image or voice is in the recording. If a person whose image or voice is in the recording is deceased, the term also means the personal representative of the estate of the deceased person; the deceased person's surviving spouse, parent, or adult child; the deceased person's attorney; or the parent or guardian of a surviving minor child of the deceased.

If you meet the criteria above you may download the Video Disclosure Request form using the link below. Once you have completed the form in its entirety you may bring it, along with the required verification documents, to the Zebulon Police.

If you would like to view a recording that depicts death or serious bodily injury, the statute requires you to complete a Request to Law Enforcement Agency to Disclose Recording of Death or Serious Bodily Injury (AOC form CV-275).

Fingerprinting Services

The Zebulon Police Department offers fingerprinting services. You must present a valid government issued identification card along with any documentation needed for the officer to sign.  These services are completed during normal business hours in between 9:00 AM - 12:00 PM & 1:00 PM - 4:00 PM Monday through Friday. 

*Please note* These cards are inked impression cards. If you need electronic / live scan fingerprint cards completed you must contact Wake County Bureau of Forensic Services located at 3301 Hammond Rd. Raleigh, NC 27610. You can go to the following website and schedule an appointment at your leisure.  (Most Child Care Applicants must have live scanned cards submitted) - Check with your resource to see which one they prefer!
